FAITH OF THE MOUNTAIN AWARD

Nomination Form

Purpose:

a) To cultivate in Maronite Young Adults an appreciation of their Maronite faith and heritage.

b) To foster leadership in Maronite Young Adults, enabling them to accept their responsibilities

in the service of God and the Maronite Church.

c) To reward, with proper recognition, outstanding Maronite Young Adults who contribute the

gifts of their time and talent to promote the programs of their local parishes.

Eligibility:

a) 18-25 years of age.

b) Previously active in MYO, where one exists and/or active membership in local Maronite

parish.

c) Current paid membership in NAM for at least one year.

(If just turning 18 - prior membership in "Family Membership" is acceptable.)

Qualifications:

a) Christian leadership in parish community.

b) Written recommendation of pastor/parish administrator.

c) The NAM Board of Directors has the option to present one at-large award per region.

1) Form must be completed by either pastor or parish administrator

2) All parts must be completed for consideration

3) Only one nomination per parish per year
